Sports Athletic Participation Packet

Student-Athletes, 

We are now going to start clearing student-athletes for the upcoming season. Please read the information below and attachments carefully as it will answer some questions you may have. The attachment will walk you through the submission process.

Requirements: 

When you create a login you must use a parent/guardian email address. This will facilitate the submission process, especially if you have siblings participating in athletics.

Physical Form

You will need to provide a completed hard copy of your Physical Form while also uploading it into Home Campus. It is required for the completed form to have a clinic stamp (your doctor will provide it after your physical upon request). If your form doesn't have a clinic stamp, you will not be cleared. 

We recommend you have your physical completed at your doctor's office or at the facility below. The facility below will take you in complimentary (free) if you book an appointment. Walk-ins are not complimentary.

All required forms, documents, and insurance cards must be properly signed and uploaded. 

Cleared or Denied Clearance 

After you complete all required paperwork you will receive an email confirmation of your submission. The athletic department will then review your documents with a decision to "clear" or "deny" you. If you are denied an explanation will be provided on what forms need to be fixed and reuploaded.
