Unified Enrollment Process (For Accepted Students)

Enrolling at Lake Balboa College Preparatory Magnet K-12

(Complete these steps only after being accepted to Magnet Program by student integration services)
Download and Complete the Enrollment Packet Documents and Materials Found Below:
Student Enrollment Form (English/Spanish/Armenian)
Student Emergency Information Form (English/Spanish/Armenian)
Student House Questionnaire (English/Spanish/Armenian)
Photocopy or scan the Following Documents:
  • Proof of Residence (utility service contract, property tax payment receipts, rental lease agreement, official government mail, current pay stub, voter registration)
  • Proof of Student's Age (birth certificate, baptismal certificate, passport, court order, health office/vital statistics record of birth certificate)
    Identification of Parent/Guardian (driver's license, DMV card)
  • Proof of Immunization - Guide to Immunization Document (English/Spanish)
If available please provide copies of :
  • transcripts or recent grade reports
  • copy of most recent IEP or Section 504 plan
