Student House Questionnaire (English/Spanish/Armenian)

Photocopy or scan the Following Documents:
- Proof of Residence (utility service contract, property tax payment receipts, rental lease agreement, official government mail, current pay stub, voter registration)
- Proof of Student's Age (birth certificate, baptismal certificate, passport, court order, health office/vital statistics record of birth certificate)
Identification of Parent/Guardian (driver's license, DMV card) - Proof of Immunization - Guide to Immunization Document (English/Spanish)
If available please provide copies of :
- transcripts or recent grade reports
- copy of most recent IEP or Section 504 plan
